TO REGISTER, PLEASE COMPLETE THE FOLLOWING FORM.

 

​

1.  Football Registration is $250.

 

2.  cheer registration is $375.

 

For multiple children in the same family:

 

a.  $25 off second child.

b.  $50 off third child.

c.  $75 off fourth child and every child thereafter.

 

3.  all registration fees must be paid prior to july 31st in order for the child to begin practice on august 1st.

UPLOAD YOUR ATHLETE'S PHYSICAL

 

Every athlete must have a physical and the doctor must complete the Pop Warner 2021 Physical Fitness & Medical History Form. Once you have the physical completed, please upload it below.

Your athlete may NOT participate in practice which begins August 1st if he/she does not have a physical.
